The Potters have been suitably impressed by Moses’ form for the club this season, with the 24-year-old contributing three goals and three assists for the team in all competitions.
Moses looks unlikely to have a long-term future as a first-team player at Stamford Bridge, with so much world class competition for places at the club at the moment.
The former Wigan Athletic man is likely to cost around £7m, and Stoke are hopeful that they can conclude a deal for the in-form wide-man.
